Hellraiser: Revival opens up with a bang, literally. It certainly doesn’t hold anything back, from its depiction of the Cenobites, to the environments you’ll be exploring. Early on in the game, after being introduced to Marlowe and his Cenobite-serving Cult, you’ll have made your way through the M359 door, under the grate as some unfortunate soul is put out of their misery, and finally gotten dressed.
You’ll then have been introduced to Bruno, via a tape right before you enter his office. If you’re here, you’re probably stuck, and wondering how to get through the mouth door, and what to do with the skull on the desk. Don’t worry, we have the answers.
How to Solve the Nail Skull Puzzle and Unlock the Mouth Door
It won’t surprise you to know that the skull on Bruno’s desk is the only way to get out of the room, and solving the puzzle will get you what you need. You’ve probably figured out from the look of the lock, and the fact it’s a mouth, that the key is something tongue related – and it is.

After you’ve finished looking around the office, and I’d suggest you do, as it gives an insight as to who Bruno is, you’ll want to head over to his desk, and you’ll find two skulls here. You can pick them both up, but for the purposes of unlocking the mouth door, you’ll want the one on the right. As you can see, there’s numerous bits and pieces to take in here, but the only thing that matters for you, is the nails sticking out of the skull. As you rotate the nail skull and move it around, you’ll notice you can interact and push the nails in.
Get them in the right order, and the nail skull puzzle is solved. Get them wrong, and it resets, and you have to start from the beginning. There’s little to indicate what is the correct order, but don’t worry, that’s why you’re here. Push the nails in, in the following order:
- 1. Left side.
- 2. Back.
- 3. Right side.
- 4. Top of the skull.
This will cause the nail skull’s mouth to pop open, and inside is the Tongue Key. Pick it up, and now you can open the mouth door. Bruno is nothing if not theme-consistent.
If you don’t know the order of the nails for the nail skull puzzle, it can quickly become frustrating, but it’s quite a smart puzzle, and definitely fits the universe. Head through the mouth door and continue on your ‘adventure’, if it can be called that.
